Comments/Questions:

Terms of the agreement include; all invoices paid within 30 days of test. Any open invoice after 30 days will be sent to collections immediately with 35% collections fee applied to grand total of account. If test is not cancelled before service attempt you will be liable for time and labor charges. Requestor hereby authorizes payment for all labor and materials for services rendered in accordance with service providers customary rates and charges. Requestor acknowledges that he/she has read, fully understands, and agrees to content of this agreement including all tests have been completed and closed tight.
